Prior to your first visit, we ask you to download the New Patient Intake Form and complete it at your leisure. Please bring the completed form with you to your first appointment.
In order to provide you with the best care possible, we ask that you take the time to complete this paperwork with detailed and accurate information about your health. Once completed, please contact us to schedule your first visit!
Your First Visit
Upon your arrival, you will be warmly greeted by our chiropractic assistant. She will request your completed initial paperwork and prepare it for your visit with the doctor.
You will then be taken to our examination room, where the doctor will begin your consultation by reviewing your health history and asking you detailed questions about your current problem. The doctor will take time to discuss your health goals and explain chiropractic.
After reviewing your history, a comprehensive physical, orthopedic, neurological, and chiropractic examination will be performed to help identify the cause of your problem. If indicated, an X-ray will be recommended to uncover structural or functional problems associated with your spine.
Upon completion of your initial assessment, the doctor will review your health history and examination findings and create an individualized care plan to get you well, which will be discussed on your next visit.
Your Second Visit
Please allow approximately 45 minutes for this visit.
Your second visit will begin with a brief video to help you better understand chiropractic care. The doctor will then begin your report of findings, which involves a discussion of your initial examination results and a review of your X-ray findings, if taken.
The doctor will explain how the findings relate to your specific problem, and then discuss treatment recommendations, as well as relevant nutritional advice and lifestyle modifications, to help you get well and stay well. Consent, scheduling and financial details will also be discussed.
During the report, we encourage you to ask the doctor any questions you may have about your health, your results, or chiropractic. Once you have a thorough understanding of your health problem and how we can help you, you will then receive your first treatment.
Following your treatment, our chiropractic assistant will provide you with information on our office procedures and schedule your future appointments. You are now on your way to achieving optimal health!
Regular Visits
Upon arrival for your regular appointments, you will always be warmly greeted by our chiropractic assistant, who will direct you to a treatment room. The doctor will then enter to treat you and address any questions you may have.
We have found that patients who take an active role in their care usually get faster results. On subsequent visits, the doctor will prescribe therapeutic exercises specific to your condition, and may offer further nutritional and lifestyle counselling. We recognize that your time is valuable, and we do everything possible to run on time and minimize the impact on your busy life.
Progress exams are performed periodically to objectively measure your ongoing success and to ensure you are healing as expected. The doctor will review your progress, and make further recommendations as necessary.
